New Patient Scheduling

ALL NEW PATIENT APPOINTMENTS SHOULD BE REQUESTED VIA THE ONLINE SCHEDULING ICON

The online scheduling icon at the bottom right corner of this page includes the next available new patient appointment openings. Follow up appointments can be scheduled at the conclusion of your new patient appointment, or by calling our office.

COST PER VISIT

  • Medical Dermatology: $125

    • At this time, we do not have contracts with insurance plans. If desired, we will provide an itemized receipt you can use to submit to your insurance for possible reimbursement of medical dermatology visits and procedures.

  • Cosmetic Dermatology: $150

    • The $150 cosmetic consultation fee will be deducted from the cost of any cosmetic procedure performed on the same day.

Scheduling Deposit

  • A $50 deposit will need to be collected before we can confirm your appointment. After we receive your online appointment request, we will send you an email with a payment link. Once we receive the payment, we will confirm your appointment. Your appointment is not confirmed until you receive an additional confirmation email.

    • This scheduling deposit will be deducted from the total cost of your visit on the day of service.

    • If you need to cancel and reschedule at least 48 hours in advance, your deposit will be transferred to your new appointment. If you cancel within 48 hours of your appointment, we will make an exception for the first occurrence, and your deposit will be moved to the new appointment. However, for a second instance of a late cancellation, and every instance thereafter, the deposit will be forfeited.

URGENT APPOINTMENTS

If you desire a sooner new patient appointment than is available via the Online Scheduling Icon, you may:

    • Request to be added to the WAITLIST by calling our office.

    • If your primary care physician or specialist is concerned that you need an expedited dermatologic evaluation, your doctor may submit an “URGENT” referral request to Avi Dermatology via eFax.

    • If your concern is emergent, please go to the emergency room at Centro Médico where dermatology is available 24/7.

Note: Our office requires a credit card on file for all patients at check-in which is used in the event of a refund due or delinquent balance.

Prior to starting, you will need:

Insurance cards (if applicable)

  • Your appointment request will be declined without insurance information unless you select self-pay. We do not yet have contracts with insurance companies, and all appointments are considered self-pay with payment due at the time of service, but we will store your insurance card to assist with prior authorizations. If desired, we will provide an itemized receipt you can use to submit to your insurance for possible reimbursement of medical dermatology visits and procedures. If you cannot find your insurance in the pre-populate list, click the “ENTER AS TEXT” option.

Access to your email

  • For security purposes, you will receive an email with a 4-digit security code to enter during scheduling. Once your appointment is scheduled, you will get an email confirmation with information on how to set up your patient portal account with Sadio. In the patient portal, you will be able to see your appointment information, update your demographic information and  medical history, sign consent forms, and see documents.

Review of Request

  • You will receive an initial email noting that our staff is reviewing your request.

*Appointment Confirmation:  YOUR APPOINTMENT IS NOT CONFIRMED UNTIL YOU RECEIVE AN ADDITIONAL CONFIRMATION EMAIL. If you have not received a second confirmation email within 3 business days, please call our office to confirm your appointment.
