New Patients
Kindly fill out the forms sent to you via the online portal prior to your initial visit with us. Please send us any prior dermatology visit notes, lab work, and biopsy results before your appointment. If you are unable to get them to us via fax or through the online portal, please bring them with you to your appointment.
Current Patients
Insurances Accepted
At the moment, we do not have contracts with any insurance companies. All services are considered self-pay. We are actively working on this. In the meantime, you will be provided with a super bill after each visit that you can submit to your insurance company for reimbursement.
